Things you may want to know are:
- This service can be as simple as providing the public with general information about the department services and/or other community agencies that have services for seniors.
- The service can also address very complex problems that seniors and caregivers have with every day living.
- The care managers are highly trained social workers who listen to the concerns of the senior or caregiver. This is followed with a thorough assessment of the problems and a discussion of a care plan to address those problems. To the extent that assistance is needed, the social worker can assist in arranging services or be an advocate to help obtain the needed services.
- Access to the Information and Assistance service can be by telephone, office visit or social worker visits to the home when needed.
- Clients have very specific rights to appropriate and professional care. These rights include the informed consent to the provision of services and the confidentiality of shared information.
