  • Difficulty with swallowing or chewing due to tooth loss or mouth pain
  • Loss of appetite, trouble shopping, buying or preparing foods
  • Involuntary weight loss caused by illness and chronic conditions

    Supplemental Meals Summary
  1. The program uses the Ensure Plus® product in flavors of Vanilla, Chocolate, Strawberry, Eggnog and Butter Pecan.

  2. Each recipient is asked to pick up the monthly supply at the Senior Services office or they may have a family member, neighbor, or friend pick up the product.
  3. Enough product is distributed to cover one meal per day for a period of one month. Two-8oz cans are equivalent to a nutritiously balanced meal. Occasionally, additional product may be provided if a person is totally reliant on the liquid protein.

A physician’s authorization must be obtained prior to enrollment in the program and renewed every six months thereafter.
