ADULT LONG-TERM CARE PLACEMENT SERVICES

   
Adult Long-term Care Placement Services provide assistance for adults who are unable to maintain themselves in their own homes independently or with available community support and need to go into Adult Care Homes (rest homes) or nursing homes.


Placement requires completion of an FL-2 Form by the client's physician.  This form indicates the level of care needed by the client and is available at physicians' offices or at  your local Department of Social Services.

 
Family members, friends, neighbors, and other agencies can aid the disabled adult in securing placement in either a nursing home or rest home. Financial assistance is available toward facility care costs if the client qualifies.
