Care Management for At-Risk Children (CMARC)

Care Management for At-Risk Children (CMARC) is a program offered at no charge for children birth toWoman and child coloring together 5 years of age who:

  • Have long-term medical conditions
  • Are dealing with challenging levels of stress
  • Are referred by the child's doctor 

Families

When the CMARC Care Manager receives a referral, they talk with the family to determine whether their child and family could benefit from CMARC services. For eligible families who want CMARC services, a CMARC plan of care is developed by the family and the CMARC Care Manager. CMARC Care Managers work with families through home visits, phone calls, visits to the doctor and other types of contact. How often CMARC Care Managers are in touch with families depends on the needs and desires of the family. 

Services

The CMARC Care Managers link children and families to community resources, medical, physical, occupational, speech and language therapy services, parenting assistance, information and education. Contact frequency is based on child and family needs and continues until such needs are met.
